During

placement which is best for you, considering the size of the implants, your
build, skin thickness, lifestyle, and other factors related to your goals
and expectations. .
The breast implant is placed either below the chest muscle (submuscular) or between the chest muscle and the breast tissue (subglandular).
Sumuscular implant placement places the breast implant under the pectoral and chest muscles. Putting the implants behind your chest muscle may interfere less with breast examination by mammogram than if the implant is placed directly behind the breast tissue. Because the muscle covers the implant, the complication of capsular contracture is less common and rippling (wrinkling) of the implant is less apparent. Also, the outline of the implant is less visible in very small-breasted women.
However with this placement, if breasts are widely spaced, it may be more difficult to create cleavage, especially with textured implants. Also, you are likely to experience more pain for a few days after surgery if the implants are placed behind the muscle than if placed directly under the breast tissue.
Subglandular implant placement places the breast implant under the breast tissue, but above the muscles in the chest. The benefits of this option include less recovery time, and significantly less discomfort and pain after the operation, since the muscles are left intact. With this placement option, cleavage is more easily created, particularly if the breasts are naturally spaced wide apart.
However, if implants are not behind the muscles, in some case, it can result in a more augmented look with a pronounced "roundness" to the breasts. This is more common especially in women with very small breasts, since there is very little tissue to help camouflage the implant. While some women may prefer this, others would rather have a more natural slope.
Also, ripples
are more easily seen and felt, especially for textured breast implants placed
over the muscle. Subglandular placement also makes doing mammograms more
difficult, and capsular contracture is more common in women with implants
placed over the muscle.
